2010 MHS/MGG Library Partnership Evaluations and Process
1. MHS/MGG events evaluation: This form is for the audience. It will be given to the audience and collected after the event. Results should be mailed to Maren Levad, MHS Library Project Coordinator, 345 Kellogg Blvd, St. Paul, MN 55101-1411. Maren will return the collected feedback to the library coordinator. Event Eval
2. Staff evaluations of an event. Evaluations will be done online. http://mhs.informz.net/survistapro/s.asp?id=3913
Results will be used by coordinators for reporting purposes and future programming and planning.
3. Follow up evaluations with performers/presenters: Maren Levad will conduct an informal phone interview of each presenter/performer of the MHS/MGG process and event outcomes, from their perspective.
4. MHS/ MGG Kits evaluation: Evaluations for the kits are included in each kit notebook. Library staff needs to give an evaluation to each participant. Results should be mailed to Maren Levad, MHS Library Project Coordinator.
5. MHS Bus Tour evaluation: When a bus tour is booked through MHS, an evaluation is sent to the library coordinator. They will need to make copies for all the participants to fill out on the return trip. The evaluations are to be sent to Jennifer Gascoigne, MHS, 345 Kellogg Blvd, St Paul, MN 55101-1411, who will share the results with Maren Levad
6. Special Workshop evaluation: Workshop evaluations will be administered by the workshop providers. The results will be shared with library staff coordinators, Ginny Brodeen and Maren Levad. Project Coordinators: maren.levad@mnhs.org
